Most compliance failures are not caused by bad intentions. They come from gaps that build up quietly until an inspection, incident or claim exposes them. Here are the five we see most often.

1. Generic risk assessments

Downloaded assessments that are never tailored to the actual site, task and people don't meet the 'suitable and sufficient' test. Review every assessment against how the work is really done.

2. Out-of-date policies

A health and safety policy signed five years ago by a director who has since left is a red flag. Review policies at least annually and after any significant change.

3. No training records

If training isn't recorded, as far as an inspector is concerned it didn't happen. Keep dated records and refresher schedules for every employee.

4. Ignoring contractors

You remain responsible for how contractors work on your premises. Check competence, insurance and method statements before work starts.

5. Not acting on near misses

Near misses are free lessons. Record them, investigate the cause and close out actions — before the same failure causes an injury.
